Parents must provide a Medication Agreement, filled out and signed by their healthcare provider if their student has a condition that will require the administration of any medication during the school day, including over-the-counter (cough drops, Tylenol, Tums, Benadryl) and prescription medications. Parents must provide medication in a original packaged or prescription container that has not expired along with the appropriately signed medication form (below). Middle School students are not able to carry any medication with them or store in their locker with the exception of inhalers and Epi pens if the appropriate paperwork has been completed.
